Ambulant myocardial ischemia and its prognostic implications.

چکیده
R ecently, unrecognized or silent myocardial ischemia has generated considerable interest. While not a new topic, silent ischemia was originally used in the context of asymptomatic or latent coronary artery disease when ischemic-type ST-segment depression was found on exercise electrocardiograms in totally asymptomatic patients. However, in the mid-1970s, when high-quality ambulatory ECG monitoring became available, many patients with coronary artery disease and relatively stable angina had frequent episodes of asymptomatic, ischemic-type ST-segment depression during daily life.1,2 Surprisingly, these "silent episodes" occurred at relatively low activity and heart rate levels compared with data from treadmill exercise tests for the same patients.23 These results suggested

Anti-ischemic therapy and stress testing: pathophysiologic, diagnostic and prognostic implications
Anti-ischemic therapy, in particular beta-blockers, is the most commonly employed drug for the control of myocardial ischemia in patients with stable coronary artery disease. Its widespread use also in patients with suspected coronary artery disease has important practical, clinical diagnostic and prognostic implications because diagnostic tests are heavily influenced by its effects. In the pre...

Imaging as an End Point in Ischemia Trials
Imaging of cardiac function and anatomy has advanced at an exponential rate over the past two decades. Our ability to quantitatively assess the degree of myocardial ischemia and accurately define the vascular anatomy using noninvasive techniques is greater than ever before. Current advances is cardiac imaging are allowing us to more safely assess patients for myocardial ischemia and better unde...
متن کاملNo-reflow phenomenon in patients with acute myocardial infarction: its pathophysiology and clinical implications.
For patients with acute myocardial infarction (MI), the immediate therapeutic goal is to establish the patency of the infarct-related artery. Prolonged myocardial ischemia, however, often breaks down the coronary microvasculature, and the flow to the infarct myocardium may be markedly reduced. This is called the no-reflow phenomenon. This phenomenon is important not solely because it correlates...
متن کاملThe detection of silent ischemia: cautions and precautions.
THERE IS NOW convincing evidence that myocardial ischemia may occur in the absence of symptoms.' This phenomenon, which has acquired the label "silent ischemia," is receiving considerable attention as a factor with potential prognostic significance in the management of coronary artery disease. Basic to an understanding of silent ischemia, its prevalence, precipitating factors, significance as a...
